I have tried multiple file types (mp4 and flv), both input variations (event and synchronized) and multiple flash output versions (10.x Worst case, I can link the video file, however I do not wish it to open in a separate instance/platform/window in order to be a seamless experience for the user. x), all to no avail.

Lectora and Lectora Online support adding captions to FLV and MP4 video formats. Click Save As and save the file as an FLV. Browse for and select the desired video file.
