The majority of people creating the courses used authoring tools, such as Authorware (difficult to learn, but robust), Dazzler Max (I used it, learning curve though), Lectora (difficult to learn) and many others that existed then. Everyone I knew, followed the ADDIE approach. Other fun tidbits AICC was the leader here, then came SCORM.

That is to say, as a whole the e-learning industry, especially authoring tools and LMS/Learning systems are often two steps back, when everyone else or at least the masses are one step forward.
