The majority of people creating the courses used authoring tools, such as Authorware (difficult to learn, but robust), Dazzler Max (I used it, learning curve though), Lectora (difficult to learn) and many others that existed then. I was one of several authors who collaborated on the survey questions and wrote different sections of the report.
