The majority of people creating the courses used authoring tools, such as Authorware (difficult to learn, but robust), Dazzler Max (I used it, learning curve though), Lectora (difficult to learn) and many others that existed then. Other fun tidbits AICC was the leader here, then came SCORM. Yep, readily available.
