Most of my clients want a list of learning objectives to appear somewhere near the beginning of the material. If I notice Im losing someones attention in class, Ill instantly make direct eye-contact, maybe raise an eyebrow, and start talking directly to that student, and most of the time. Im the same when teaching face-to-face -- big user of the laugh to create connection. There are many VoIP tools that allow sending video as well as audio.

In Rapid Project Management Techniques for e-Learning presented by Coates & Hill of Deloitte, they outlined their approach to creating a big enterprise-wide training program in an extremely aggressive timeframe.
